How long does PrEP take to work?
PrEP for HIV needs time to build up before it protects you. With daily pills and no loading dose, that is about a week for receptive anal sex and about three weeks for receptive vaginal sex.

Daily PrEP pills: the CDC lead-in times
The CDC’s guide to PrEP gives these times for daily pills to reach maximum protection:

Starting PrEP with a double dose
The 2025 BHIVA/BASHH PrEP guidelines from the UK say oral PrEP can be started with a double dose (two pills) 2 to 24 hours before sex. They apply this to all types of sex and to injecting drug use, mostly based on drug-level studies.

How long does PrEP take to work for different types of sex?
Under the UK guidelines, how long you continue after the last sex differs:

The evidence for receptive vaginal sex is rated lower. WHO does not recommend 2-1-1 for vaginal sex, so daily PrEP is the usual choice, as our comparison of PEP and PrEP explains.

How long do PrEP injections take to work?
Long-acting PrEP injections are not available everywhere. The CDC says their lead-in time is not yet known, while UK guidelines say injectable cabotegravir is protective 7 days after the first injection.

How long PrEP keeps working after you stop
The UK guidelines advise one pill a day for 2 days after the last anal or insertive vaginal sex, and for 7 days after receptive vaginal sex or injecting.

HIV PEP Bangkok, near BTS Asok and MRT Sukhumvit, is open every day from 09:00 to 21:00. A PrEP visit includes screening, a kidney check and a 30-day supply, and a rapid 4th-generation HIV test gives results in about an hour.

The CDC has no data on lead-in times for insertive sex. UK guidance uses the same double-dose start and 2 more days of pills after the last sex.
Do I have to wait again if I restart PrEP after a break?
Usually you start again as you did the first time, often with a double dose. If you had sex during the break, ask a clinic whether you need PEP or an HIV test first.
